Step 1: Initial Consultation

We begin with a no-pressure consultation, offered in-person or via video call, where we gather key details about your loved one’s health, lifestyle, and needs to establish a foundation for personalized care.

Step 2: Comprehensive Assessment

Our care coordinators perform a thorough assessment, either in-person or virtually, involving family input to evaluate the client’s environment, routines, and safety concerns for a tailored care approach.

Step 3: Care Plan Development

We collaborate with the client and family to create a customized care plan, integrating medical history and preferences, accessible via our secure family care portal for ongoing adjustments and communication.
